SAFETY RECALL

Follow these instructions to order replacement brackets for Simmons Juvenile Products cribs.

 

Shown here are several examples of Simmons "caution" labels, where the model number, year code and date code can be found:

Your crib model number will be designated as a four digit or a six digit number. The date code is indicated as a three digit number and the year code with a two digit number. The labels to the left show several examples of how a crib made in 1998 could be referred to on a label. It is important that you list each number just as it appears on the label.

ORDER FREE REPLACEMENT BRACKETS

Please fill out this form completely, to be certain the correct replacement corner brackets will be sent to you. If you have more than one crib, please place an order for each crib. Immediately after submitting this form, a CONFIRMATION NUMBER will appear on your screen. Save this number for reference purposes.

Cribs with year codes other than 98, 99 and 00 are not subject to recall. If you have questions, call (800) 421-2951 to speak to a company representative.
